Schedule for Well Child Checks and Vaccinations 

Birth                     Hepatitis B in hospital

3 - 4 days            Newborn Check in office - RSV

2 weeks               Well Check, weight check

1 month               Well Check, weight check

2 months             Well Check – Pediarix (DTap, HepB, IPV), PCV13, RV, Hib

4 months             Well Check – Pediarix (DTap, HepB, IPV), PCV13, RV, Hib

6 months             Well Check – Pediarix (DTap, HepB, IPV), PCV13, Hib, Flu #1 (Flu #2 due in 1 month)

9 months             Well Check – Developmental Check, (Flu #2 if needed)

12 months           Well Check – MMR, Varicella, PCV13, Hep A, (Flu #2 if needed), Hemoglobin and lead

15 months           Well Check – Hib, Dtap, (Flu if needed)

18 months           Well Check – Developmental check, Hep A, (Flu if needed)

24 months           Well Check – Developmental check, Hemoglobin and lead, (Flu if needed)

30 months           Well Check – Developmental check, (Flu if needed)

3 years                 Well Check  - Developmental check

4 years                 Well Check – Pre K evaluation, MMR, Varicella, Kinrix (DTaP, IPV), (Flu if needed)

5 to 9 years         Well Check – with yearly Flu vaccine in fall

10 years               Well Check – HPV, Flu vaccine in the fall

11 years                Well Check – HPV, Tdap, MCV4, (Flu)

12 to 15 years      Well Check – with yearly Flu vaccine in fall

16 years               Well Check – MCV4, MenB (MenB #2 due 1 month later), (Flu)

17 to 21 years      Well Check – with yearly Flu vaccine in fall

Key to abbreviations:

  • DTaP: Diphtheria, tetanus, and acellular pertussis vaccine

  • Flu:  Seasonal influenza vaccine - quadrivalent

  • HepA: Hepatitis A

  • HepB: Hepatitis B

  • Hib: Haemophilus influenzae type b vaccine

  • IPV: Inactivated poliovirus vaccine

  • MCV4: Meningitis ACWY

  • MenB: Meningitis B

  • MMR:  Measles, Mumps, Rubella

  • PCV13: Pneumococcal conjugate vaccine
